Hospice RN Case Manager

If you answered "yes" to the above questions YOU may be the RN Hospice of Redmond is looking for to lead its special team of hospice professionals as Case Manager. This position is responsible for the case management of Hospice of Redmond patients, working along side Redmond’s interdisciplinary team of RNs, CNAs, social workers, and chaplains providing high quality end-of-life care to our patients and their families. 

Our Hospice of Redmond RN Case Manager responsibilities include overseeing the ongoing, individualized Plan of Care for each patient to assure it meets the patient’s needs; maintaining accurate and timely patient records for completeness and regulatory compliance.

The successful candidate will have knowledge of principles and practices of primary medical and nursing care with a working knowledge of end-of-life and palliative care;  and knowledge of the process of dying and bereavement. Two years of progressive nursing  experience, hospice or home health experience preferred but not mandatory.

This full-time position offers excellent benefits. Compensation commensurate with proven experience and abilities.

Qualifications: Oregon RN License, Valid Driver’s License; Reliable transportation and vehicle insurance.

Standard Schedule:  4 day work week.  One on-call night per week, one on-call weekend per month.
