Hospice Social Worker (MSW)

Hospice of Redmond invites you to become part of our team of dedicated professionals providing quality end of life services to the residents of Central Oregon. If you have a Master’s Degree in Social Work and can provide holistic, family-centered care honoring a patient’s and their family’s unique spiritual and cultural beliefs, this may be the position for you.

Our successful candidate will be a vital part of our interdisciplinary team, collaborating with other care team members to establish, review and revise the plan of care for each patient. Primary responsibility for this position is the development, implementation, and management of psycho-social services provided to hospice patients and families. Requires strong assessment skills of patient/family social and emotional factors, ability to provide counseling and emotional support, and assist patient/family and staff in accessing community resources. Must be able to work independently, have demonstrated patient care planning and patient teaching skills, along with strong written/verbal communication skills.

We are looking for a Certified Nursing Assistant (CNA) to join our caring and dedicated hospice team. This position is full-time and includes excellent benefits. NO WEEKENDS & NO ON-CALL!!! Candidates must possess strong assessment and communication skills, be able to work autonomously and have a heart and passion for the position. Hospice RN Patient Care Coordinator

Are you creative, have sound judgement, excellent nursing skills, work well under pressure, customer-oriented, and a relationship builder?  You may be the RN Hospice of Redmond needs to lead its special team of hospice professionals. This position is responsible for the supervision and coordination of the Hospice of Redmond’s interdisciplinary team of RNs, CNAs, social workers, chaplains, and therapists providing high quality end-of-life care to our patients and their families.

Our PCC responsibilities include overseeing the ongoing, individualized Plan of Care for each patient to assure it meets the patient’s needs; monitoring patient records for accuracy and completeness for regulatory compliance; and takes a leading role in the QAPI hospice performance improvement program.

The successful candidate will have knowledge of principles and practices of primary medical and nursing care with a working knowledge of end-of-life and palliative care; the ability to build and lead an interdisciplinary health care team; and knowledge of the process of dying and bereavement. Two years of progressive nursing management experience including direct supervisory responsibilities needed. Hospice or home health experience preferred but not mandatory.
